Free COVID-19 Vaccination Clinic for Bedford Youth ~ Ages 12 to 18 ~ Saturday, May 22 ~ 10 am to 1 pm


This Saturday, May 22, the Bedford Health Department is offering a FREE COVID-19 vaccination clinic. Bedford Public School students and/or Bedford residents aged 12-18 are eligible for appointments. The Pfizer vaccine will be offered, and 400 appointments will be available. Attendance at the clinic is by appointment only. Please do not arrive before your appointment time.

To register for an appointment, navigate to

Choose an appointment time. Click CONFIRM.

Enter the name of person being vaccinated and a contact email address then click the “Yes” box to complete the Consent Form and click SCHEDULE EVENT. You will receive an email confirming the date, time, and location of your appointment.

Fill in the first page of the registration consent form (Massachusetts required vaccination information tab) with the requested personal information for the person being vaccinated. Click NEXT.

Select your current health insurance status. If requested, upload a picture of the requested health insurance information. Click SUBMIT.

You will then receive an email with a user name and password with a link to print the consent form. Navigate to the link and enter the requested information.

Click PRINT CONSENT FORM. This must be printed before coming to the clinic.

On the day of the clinic, answer the medical screening questions on the Consent Form and complete the form. Sign and date the Consent Form.

Please BRING THE SIGNED CONSENT FORM TO THE CLINIC. We will be unable to vaccinate those who arrive at the clinic without signed consent forms. A form is required for each person to be vaccinated.
